CommunicareKeeping rural communities well

Are you supporting a family member or friend who may be suffering from an undiagnosed Mental Health issue such as depression, anxiety, feelings of being overwhelmed or having difficulty getting through the day?

Then CommuniCare5000 is the program to support you in your caring role, working hand in hand with the carer to tailor a plan of action to meet the individual needs, or in the case of a family this could be individual plans for each member of the family affected.

What is CommuniCare5000 ?

CommuniCare5000 is a program to support individuals and families affected by Mental Health issues. Support is provided through the use of Case management, a caseworker will assess your situation and work out how the program may best assist you. In the case of family members and carers, a plan will be worked out by a caseworker to make sure the service you receive is best suited to your needs. Caseworkers will stay in touch with you to ensure that the help that has been provided continues to meet your needs, and whether any further supports may be needed.

Service funding can also be provided to assist the family or friend to gain access to services or emergency assistance. Referrals can be made to services both locally and non-locally such as referrals for respite to the Commonwealth Carelink and Respite Centre to access the many different kinds of assistance available to people caring for another.

Who can get assistance?

If you are caring for or living with someone who has a Mental Health issue, or maybe you are the neighbour or a friend supporting someone with a Mental Health issue then assistance is available to you. Assistance is available to both the individual or family including parents, children, brothers or sisters.

Assistance is also available to communities through providing education and better awareness for community members about Mental Health issues.

Where is CommuniCare5000 assistance available?

CommuniCare5000 is available throughout the Riverina Murray, with services designed to meet the needs of families and carers who live in communities with less than 5000 people.
