Community Options
Straightforward support for complex needs
Are you elderly or living with a disability or caring for someone that is? Do they have multiple or complex care needs? Complex care needs can be needs such as multiple disabilities, an unstable condition requiring monitoring and flexible rapid service changes, inadequate community supports, financial disadvantage, no independent community access, dementia, geographical isolation or a carer that is ill, frail or stressed.
If so, then Community Options may be able to assist you.
What is Community Options?
Community Options is a case management and brokerage service for people with complex and changing needs.
A Case Manager is someone who organises the services, supports and assists you to link with appropriate services and can advocate on your behalf. Your case manager will get to know you and plan services with you, you will also be involved in regular reviews of your care plan, until your needs are met and you exit the program. Your case manager is there to ensure that you can continue to live as independently as possible within the community.
Brokerage is when services are purchased on behalf of the client or the carer, in some instances this may mean that there is very little or even no cost to the carer for the provision of the service.
Please note: Funding for clients in the Community Options program is not permanent or ongoing.Who can access Community Options?
Community Options is available to people with difficulties caused by age or disability and who have complex care needs.
What does it cost?
Currently there is no charge for case management services, however you may be asked to pay a contribution towards services arranged for you. We will discuss all fees with you before implementing your care plan.
Where is Community Options available?
Community Options are available in the local Government areas of Deniliquin, Conargo, Wakool, Murray, Berrigan and Jerilderie.
