Community Options ComPacks

ComPacksA safe and supported return home

ComPacks is a program which aims to improve access to the community services that patients may require for a safe and supported return home from hospital. ComPacks is a case-managed discharge from hospital and it includes a package for care for up to six weeks after discharge.

How does ComPacks work?

Whilst in hospital, staff will assess and refer patients to a ComPacks service provider, then a Case Manager will complete a detailed assessment on the patient. Before the patient is discharged from the hospital a ComPacks case management plan will be put in place to assist with services such as personal care (showering, dressing, grooming), domestic assistance (house cleaning, laundry, shopping) or other services such as meals, social support, transport, respite care, personal alarm systems, carer support or centre-based day care.

Who is eligible for ComPacks?

Patients of participating hospitals (Deniliquin, Berrigan and Finley Hospitals), and those who have been assessed as needing two or more services on discharge from hospital and those who require ComPacks to be discharged.

People who are not eligible are people who are currently receiving a Community Aged Care Package (CACP), people who are on a waiting list for nursing home placement, people who are waiting for Attendant care funding or people who only require one service.

Where is ComPacks available?

ComPacks is run by Deniliquin Community Options and referrals are made from the Deniliquin, Berrigan and Finley Hospitals.
