Your steps to securing in-home aged care support.

By following these steps, we aim to provide you with a seamless transition to a life enriched by personalised care.
Step 1: Assessment and eligibility
You will first need to ensure you are eligible for a My Aged Care support, such as a Home Care Package.
Home Care Packages (HCP) are a government-funded program designed to support older Australians to live independently in their own homes.
Eligibility for the program is based on factors such as age, income, and level of need.
To get started, you must register with My Aged Care (MAC).
- Phone 1800 200 422 and discuss your care needs and register.
- Go to myagedcare.gov.au/accessible-all if you have specific communication needs.
Feel free to reach out to our friendly Intereach team who will guide you through the process to help guide you with your eligibility assessment application.
During your initial chat our Intereach Aged Care Services team member, they will ask you some basic information about yourself.
This will determine whether you are eligible for an HCP Assessment and could be used to support you during the referral process to My Aged Care.
If you are eligible, an independent Aged Care Assessment Team/Service will carry out a personalised assessment to determine your eligibility and Home Care Package Level.
Step 2: Choose Intereach as your home care provider
Once you have been assessed and are eligible for aged care services, Intereach is your next point of call.

Step 3: Choosing the right package
Once your package is assigned, we work with you to understand your goals and needs, so that we can create a personalised care plan just for you.
Forget one-size-fits-all. Our Home Care Packages are as unique as you are.
They are designed with you, to fit your lifestyle so you can focus more on what brings you joy. Choose the services that matter most. The types of support you may be eligible for include assistance with housekeeping, maintaining your garden, or enjoying social outings. We offer a wide range of support options so you can do more of what you love.
You have the freedom to spend your allocated funds on most types of services, support, and equipment related to your care, health and wellbeing. This means you can truly personalise your plan and make it work for you.

Step 4: Starting your services
With your plan in hand, it’s time to bring your personalised services to life.
Your plan is a living, breathing document, evolving as your needs change. We are committed to empowering you to do more, be more and live more at home for longer.
As your Home Care package commences, you will meet with our Care Coordinators who will make sure everything goes to plan. You will also meet any of the expert, friendly team members who will be coming into your home to provide support, so you can get to know them, and they can get to know you and your home.
With Intereach, you can do more. A Home Care Package with us also means you have the advantage of being able to use some additional free supports like our volunteer visitors and social support volunteers. And, depending on where you live, you may also be able to access Community Transport and Meals on Wheels*. Chat with our Aged Care Team members for more information.
