What can I expect from my aged care support team?
Your needs will guide everything the Intereach aged care team delivers to you.
Week to week, you will come to recognise a familiar set of faces – these are the skilled and trusted Community Support Workers and Domestic Assistants providing the services you need to remain independent at home and connected in your community.
Behind the scenes is a dedicated team that coordinates your supports, rosters staff and responds to any queries you may have.
You are in control at all times. If you have a question, some feedback or need to make a change, we’re here to help you.
Not sure what a Community Support Worker does?
Meet Sally, Sally is a Community Support Worker. She visits clients in their homes to provide support with meal preparation, personal care, and cleaning.
It’s a role she takes great pride in.
“Every day I get to work with people who have amazing life experiences and wonderful stories to tell,” Sally said.
“They appreciate the time I take to check-in with them while I make sure their needs and expectations are being met.
“I’m not there to take over. I’m there to support them.
“Sometimes I complete the parts of a task that my participant can no longer do. At other times, I help them to regain their confidence in doing particular activities. We call this re-ablement.
“For example, a participant of mine was experiencing back pain after surgery and found it difficult to stand in their kitchen to prepare a meal.
“So, I worked with them to establish some alternatives – like sitting at the kitchen bench and working in short bursts – which enabled them to move, rest, and relieve their pain while continuing to cook for themselves, eat well, and remain independent.
“Another participant was recovering from a fall and was worried about getting to their clothesline to put their washing out. I noticed the need for some home modifications to improve their safety and let their care partner know. I then helped my participant to make use of drying racks closer to their backdoor while they steadily regained their confidence.
“I’ve been working with a participant who needs a little extra support getting around town. I’ve reached out to our Community Transport team, and they will soon begin accessing this helpful service.
“One of the best things about my job is seeing participants feel confident again. They may not be able to do all the things they once did, but they know they can stay on top of things at home and remain connected in their community because they have the support that they can trust,” she said.
