Support at Home is coming — 1 November 2025
From 1 November 2025, the new Support at Home program will replace Home Care Packages (HCP). It will make getting in-home aged care simpler, more flexible, and easier to tailor to your life.

How it works
Step 1: Get in touch
Call 1300 488 226, email agedcare@intereach.com.au or enquire online. We’ll listen, answer your questions, and explain the options.
Step 2: My Aged Care & assessment
We’ll help you register with My Aged Care and prepare for your assessment. The you will be matched with an assessor who will talk with you about your goals, safety and support
Step 3: Choose your service
Support at Home offers a wide range of services to help you keep living independently at home. Once you’ve had your aged care assessment and your eligibility is confirmed, you’ll be able to access a list of approved services. We’re here to help you choose the mix of services that best suits your needs and fits within your budget.
Step 4: Start your personalised plan
We coordinate services, schedule visits and check in regularly. As your needs change, your plan can change too—your place, your pace.

What will Support at Home change?
It will gradually bring CHSP and HCP into a single, more flexible program. You’ll still have choice and control, with support tailored to your goals.
Am I eligible?
Eligibility is determined through My Aged Care based on your needs and circumstances. We can guide you through the steps.
What will it cost?
Government-subsidised services usually include a contribution based on your situation. We’ll explain fees upfront so there are no surprises.
Can family or carers be involved?
Absolutely. We work alongside families and carers and respect your preferences and cultural needs.
